Cardiocom’s telehealth system proactively manages and motivates the members towards self care with interactive Health Check and collection of vital signs including: weight, blood pressure, glucose and heart rate. Through standard phone line or cellular connection, the information will be immediately transmitted to the participant’s nurse coach at Focused Health Solutions Deerfield, IL based clinical nurse call center. The participant’s nurse coach reviews the information with the participant during their scheduled condition management education calls throughout the year.
