US-based healthcare provider Carbon Health has acquired remote patient monitoring (RPM) company Alertive Healthcare for an undisclosed sum.
Alertive Healthcare’s advanced platform of RPM tools will help providers to manage patients proactively across various specialties ranging from primary care, cardiology, neurology, and nephrology.
The company’s platform is used to record physiological data from patients and send intelligent alerts to providers to streamline documentation and decrease treatment time for critical chronic conditions.
The acquisition of Alertive Healthcare will allow Carbon Health to boost its omnichannel care delivery strategy, as well as expand its home-based care capabilities and remote patient care offerings.
Carbon Health home-based care president and chief strategy officer Myoung Cha said: “Carbon Health has already seen the power of RPM tools throughout the COVID pandemic in enabling better care for patients remotely with blood pressure cuffs, glucometers, and scales.”
Alertive automates medication management to avoid heart attack and strokes before they occur by enabling the patient and doctor know when the patient’s blood pressure is rising.
The company’s platform and services will be integrated into Carbon Health’s omnichannel care model over the next several months.
The integration will allow Carbon Health to extend its home-based care capabilities with connected devices, wearables and other sensors to share patient’s vitals with its provider.
Carbon Health providers will also have real-time visibility into patients’ vitals to alert and monitor patients at home.
The company will also continue to work with Alertive Healthcare’s provider customers to serve a range of patients for post-acute care and chronic condition management.

In June this year, Carbon Health acquired a fully integrated diabetes platform Steady Health for an undisclosed sum.
