The EHC remote patient monitoring solution will provide required updates of patient data to healthcare providers to oversee the progress of their patients, allowing for improved patient care.

Healthcare providers can view each patient’s vital statistics and make adjustment to care plans, which is then communicated to the patient via the EHC device.

The EHC touch screen device will deliver automated reminders and increase patients’ compliance with care plans, diets, medication schedules and follow-up physician visits.

The system can even educate patients about the disease management and can also be used for patients who require ongoing management of diabetes, hypertension and sleep apnea.

Meadville chief innovation officer Barry Bittman said they are using the remote monitoring program to improve chronic condition outcomes and ongoing care for their patients after discharge from the hospital.
