Ochsner Digital Medicine, a connected health programme offered by Ochsner Health, has agreed to provide digital medicine services to the members of AlohaCare.
AlohaCare is a Hawaii-based community-led, non-profit health plan that currently serves around 80,000 Medicaid and 2,300 Medicare members.
Ochsner Digital Medicine will provide digital programmes to AlohaCare members with Hypertension and Type 2 diabetes, to help remotely manage and control their disease.
The programmes will enable remote patient management through digital devices and personalised plans to help AlohaCare members live healthier lives.
In addition, the programmes will help with medication management, lifestyle modifications, progress tracking and motivational support.
Each member will be assigned a licensed clinician and professional health coach to serve as an adjunct to their primary care provider.
The partnership will enrol members and provide digital medicine services immediately from 1 September 2023, the effective date of the partnership agreement.

Ochsner Health is an integrated healthcare system, with over 80 years of service in cancer care, cardiology, neurosciences, and liver and kidney transplants, among other areas.
Its Connected Health Digital Medicine programme applies technology and data to create customised, patient-centred digital health solutions to help manage chronic conditions.
The programme helps patients with hypertension and Type 2 diabetes to manage their health, improve health outcomes, and reduce healthcare costs.
Ochsner Health said around 80% of participants enrolled in its Digital Medicine programmes achieve control of their condition within six months.
